Agreement and Consent
Upon arrival on your first appointment with Sutton Sports Medicine you will complete a consent form whereby you are consenting to being assessed and treated by the corresponding therapist and agreeing to the Terms & Conditions. If you are a parent, guardian, or career for the patient, you will be consenting on behalf of the patient, and will personally be bound by these Terms & Conditions. Please read these Terms & Conditions carefully. If these Terms & Conditions are updated you will be notified on these changes either in person whilst in the clinic, via email, or via notices in the clinic.
Throughout your time with us at Sutton Sports Medicine, the therapist will not carryout any form of objective assessment or treatment without your consent. All decisions are made by a shared-decision process.

Payments
You can see the current treatment prices on our website. Prices may differ from clinic appointments and home appointments. Any changes to treatment prices you will be notified, either via email, verbally in clinic, or via notices in clinic. Any discounts and promotions will be relayed in a similar manner. Discounts cannot be used in conjunction with other discounts and promotions. Invoices can be provided upon request.

Home Appointments

When paying for your home appointment you can either pay in cash or pay via bank transfer. We do not have a card machine currently; this may change in the future, in which you will be notified of.
2.3 Late Payments & Outstanding Charges
If you have any outstanding chargers, or cannot pay on the day for unforeseen circumstances, a charge will be raised on your account. This charge will need to be paid on your next appointment or within 7 working days. Failure to do so may lead to additional administrational charges.

Cancellations and Non-attendance
The full responsibility of attending your appointment is yours. Please be aware of our 24-hour cancellation policy. We reserve the right to charge 100% for appointments cancelled within the 24-hour cancellation and for those who do not attend their appointment without making us aware. A charge will be raised on your account and an invoice will be sent to you via email.

Confidentiality and Medical Records
Sutton Sports Medicine is committed to maintain strict confidentiality with regards to medical records and personal information. We agree to provide access to your medical records on receipt of a signed and fated medical records access request under the Data Protection Act. We may request, with your consent, further medical records from your GP in order to ensure you receive optimal treatment while you are with us.

Contact Details
It is important that you keep your provided contact details up to date. We are not liable should you fail to update us regarding any changes to your contact details, including name, email address, telephone numbers and/or address. We do not share your contact details without consent, please see our Private Policy.

Personal Possessions
Whilst you are in the clinic, your personal possessions are your responsibility. Sutton Sports Medicine is not liable to any theft, loss of, or damage to your personal property.

All communication made through external social media platforms that the owners of Sutton Sports Medicine on are custom to their own Terms and Conditions and privacy policies they hold. Please be aware of this, use these platforms wisely and communicate on these with care and caution with regards to using personal and medical information.
